Job summary

Hueman is recruiting Nurse Practitioners to conduct in-home Health Risk Assessments for Medicare and Medicaid members near Campobello, SC. This role offers a flexible schedule with travel to patient homes and comprehensive home assessment training, supported by dedicated scheduling assistance.

Qualified NPs will have a Master’s degree, an active NP license, and board certification with current BLS. Experience is preferred but not required, and liability coverage is provided.

Qualifications

• Master’s degree in nursing or commensurate clinical experience.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of experience as a paid advanced clinical provider preferred but not required.

Responsibilities

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate and timely documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative healthcare measures.
