Hueman is hiring Nurse Practitioners to perform in-home Health Risk Assessments (HRAs) for Medicare and Medicaid members around Prospect, KY. You will travel to patients’ homes and conduct comprehensive, non-invasive physical assessments in a setting that supports work-life balance.

NPs are not required to treat or prescribe in the home. Qualified candidates hold an Active State NP license, National board certification, BLS, and a Master's degree (or equivalent experience).

Qualifications

• Master’s Degree or commensurate experience must be met.
• Active State NP license.
• Active National board certification.
• Current BLS certification.
• 12 months of paid advanced clinical provider experience preferred but not required.

Responsabilités

• Travel to and from patients’ homes.
• Perform comprehensive, non-invasive physical assessments.
• Review medical history and ensure accurate documentation.
• Conduct medical reconciliation to ensure patient safety and compliance.
• Provide basic patient education on wellness and preventative measures.
