Job summary

Source One Management Services, LLC seeks Nurse Practitioners and Physician Assistants for flexible PRN in-home Health Risk Assessments (HRAs) for Medicare Advantage members. Build your schedule with an 8-day monthly minimum, enjoy competitive pay with incentives, and document easily using a company-provided iPad.

No prescribing, no on-call shifts, and malpractice insurance are included; you’ll travel locally within your county and nearby areas to complete HRAs.

Qualifications

• Active NP or PA license required (excluding Pediatrics, PMHNP and Women’s Health)
• Minimum availability of eight full 7.5-hour days per month
• Willingness to travel within roughly 60 minutes of your home

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients
• Review patient history, current medications, lifestyle, and key social factors
• Conduct basic physical and mental health evaluations
• Travel locally to patients in your county and nearby areas
