Job summary

Source One Management Services, LLC seeks Nurse Practitioners and Physician Assistants for flexible in-home Health Risk Assessments (HRAs) for Medicare Advantage members. This PRN contractor role offers autonomy, a simple iPad-based documentation process, and malpractice insurance.

Eligibility requires an active NP or PA license and a willingness to travel roughly 60 minutes from home. Expect eight 7.5-hour days per month with competitive pay and incentives.

Qualifications

• Active NP or PA license required (excluding Pediatrics, PMHNP and Women’s Health).
• Willingness to travel within roughly 60 minutes of your home.
• Minimum availability of eight (8) full 7.5-hour days per month.

Responsibilities

• Complete comprehensive in-home HRAs for Medicare Advantage patients.
• Review medical history, current medications, lifestyle, and social factors.
• Conduct basic physical and mental health evaluations.
• Travel locally to patients in your county and nearby areas.
