Job summary

Compassus, in collaboration with Ascension at Home, is hiring a Home Health Physical Therapist to deliver in-home care. You will work one-on-one with patients in their own homes, helping them regain strength and independence through personalized therapy plans and hands-on treatment.

Minimum qualifications include a Bachelor of Science in Physical Therapy, current state license, valid driver’s license, CPR, and two years of PT experience in acute care or rehabilitation.

Qualifications

• Bachelor of Science in Physical Therapy.
• Current State License as a Physical Therapist.
• Valid Driver’s License; CPR, TB screening and Hepatitis consent/declination.
• Two years’ experience as a Physical Therapist in an acute care or rehabilitation setting.
• Knowledge of durable medical equipment.

Responsibilities

• Provide in-home PT assessments and treatments to patients in their own homes.
• Develop and adjust individualized therapy plans to restore function.
• Collaborate with patients and families to maximize independence in daily living activities.
