Compassus is seeking a Hospice Registered Nurse / RN Case Manager to provide patient- and family-centered hospice care in the home setting. You will coordinate with physicians, social workers, and other team members to ensure seamless, compassionate care that honors patients’ goals and comfort needs.

This role requires an active RN license in Texas, a current CPR/BCLS certification, and strong communication skills to support families through difficult times.

Qualifications

• Active RN license in the state of employment.
• Minimum two years of RN experience; hospice experience preferred.
• Current CPR and BCLS certification.

Responsibilities

• Provide hospice care that supports quality time with loved ones.
• Collaborate with the care team to coordinate in-home care plans.
• Educate families and empower caregivers to maintain comfort and dignity.
