Qualifications

• Active Florida Physical Therapist (PT) license.

Responsibilities

• Evaluate and treat residents in home or ALF settings.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the care team.
• Maintain accurate, timely documentation in the EMR.
