Job summary

Sobe Innovative Rehab is seeking a licensed Physical Therapist (DPT/PT) to provide Hospice services in patients' homes in Hudson, FL. This PRN position offers flexible assignments and the chance to support older adults in maintaining independence and quality of life.

Responsibilities include evaluating residents in home or assisted living settings, developing individualized treatment plans, collaborating with physicians and families, and maintaining EMR documentation with clinical autonomy

Qualifications

• Florida PT license required.
• Degree in Physical Therapy from an accredited program.
• New graduates encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and or Home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
