Job summary

Sobe Innovative Rehab seeks a licensed Physical Therapist to provide Hospice care in a home setting in Port Saint Lucie, FL. This PRN role focuses on compassionate, evidence-based therapy to help older adults maintain independence and quality of life.

Responsibilities include evaluating residents, developing treatment plans, and coordinating with care teams. Florida PT license required; new graduates are welcome to apply. Flexible assignments and competitive pay offered.

Qualifications

• Active Florida Physical Therapist license.
• Bachelor's, Master's, or Doctorate in Physical Therapy from an accredited program.
• New graduates and experienced therapists are encouraged to apply.

Responsibilities

• Evaluate and treat residents in an assisted living community and/or home setting.
• Develop and implement individualized treatment plans.
• Collaborate with physicians, families, and the interdisciplinary care team.
• Maintain accurate, timely documentation in the EMR.
