Job summary

Option Care Health, Inc. is hiring an Infusion Nurse II to coordinate and deliver intermediate direct patient care in the home or an alternate infusion suite, ensuring patient safety under a physician's plan of care.

Working under supervision, you initiate and adjust treatments, educate patients and families, document care in real time, assist with discharge planning, and collaborate with the clinical team to maintain high-quality, compliant care.

Qualifications

• Licensed registered nurse (RN).

Responsibilities

• Initiates, develops, and implements intermediate nursing plan of care treatments, evaluating patient progress towards goals.
• Assesses patient needs and physical status at each skilled visit; re-evaluates and clarifies physician orders; informs physician of changes.
• Provides training and mentorship to Infusion Nurse I's to ensure patient safety and compliance.
• Provides effective teaching using patient-centered care to achieve independence with prescribed therapy and care needs.
• Communicates with the Clinical team, medical providers, patients, and families to facilitate continuity of care; documents in real-time.
• Coordinates discharge planning and prepares discharge summaries with patient instructions.
• Demonstrates compliance with agency policies, procedures, standards, and regulations.
• Maximizes work efficiency through computers to validate plan of treatment orders and document visits.
• Responds promptly to patient requests and initiates emergency procedures when necessary.
• Follows HIPAA and protects patient and company information.
• Speaks knowledgeably about options of care and explains related financial obligations.
• Attends required training modules and continuing education to maintain competency; may supervise paraprofessionals.
• Participates in multidisciplinary team conferences.
