Job summary

Jethro Health System LLC seeks a compassionate Physical Therapist or Physical Therapist Assistant to provide in-home care. You will assess, treat, and coach patients in their homes, following a planned care program, and coordinate with families and other health professionals.

The role offers part-time or per-diem hours, Monday to Friday schedule, with 10–30 hours per week expectation, and a focus on maintaining patient independence.

Qualifications

• Associate's degree in Physical Therapy is required.
• Massachusetts Physical Therapy Assistant license is required.
• CPR Certification is required.

Responsibilities

• Provides care and services in accordance with the plan of care.
• Assists community members and health field personnel to assess, plan for, and provide needed health and related services.
• Documents care provided in an accurate and timely manner.
• Provides teaching to clients and families.
• Coordinates and collaborates with other members of the health care team, community agencies, and referral sources.
• Communicates the plan of care with patients and families and ensures understanding of the exercise program.
• Promotes a positive image of the company and meets patient expectations.
• Adheres to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures.
• Attends continuing education programs.
• Other duties assigned by the department manager.

We are looking for a compassionate, reliable PT and PTA to join our team to provide excellent care and who enjoys working with patients in an at-home setting.

Responsibilities

• Provides care and services in accordance with the plan of care.

Requirements/Qualifications

• Associate's degree and graduation from an accredited Physical Therapy program.
• One year of physical therapy experience preferred.
• Possesses and maintains CPR Certification
• Possess valid driver's license
