Job summary

Jethro Health System Company is seeking a Part-Time or Per Diem Physical Therapist (PT) or Physical Therapist Assistant (PTA) to perform home health visits in Massachusetts. The role focuses on patient independence and quality of life in the home setting.

The successful candidate will have an accredited PT program degree, MA licensure, CPR certification, and a valid driver’s license. Typical hours range from 10–30 hours per week with potential flexibility for part-time or per-diem engagement.

Qualifications

• Associate's degree and graduation from an accredited Physical Therapy program.
• One year of physical therapy experience preferred.
• Possesses and maintains CPR Certification
• Possess valid driver's license

Responsibilities

• Provides care and services in accordance with the plan of care.
• Assists community members and health field personnel to assess, plan for, and provide needed health and related services.
• Documents care provided in an accurate and timely manner.
• Provides teaching to clients and families.
• Coordinates and collaborates with other members of the health care team, community agencies, and referral sources.
• Communicates the plan of care with patients and families and ensures understanding of the exercise program prescribed.
• Responsive to meeting or exceeding patient/client expectations and promoting a positive image of the company
• Adheres to HIPAA laws, maintains patient confidentiality, and adheres to all agency policies and procedures.
• Attends continuing education programs.
• Other duties assigned by the department manager.
