Job summary

Lifespan Therapy is seeking a Home Health Physical Therapist (PT) to provide skilled in-home care for geriatric patients across California. You will conduct comprehensive evaluations, develop individualized plans of care, and work with a supportive team to ensure optimal patient outcomes.

Qualifications

Valid California Physical Therapist license.
CPR or BLS certification.
Driver's license and auto insurance.
Physical + TB test (within the last year).
Professional liability and business license (if applicable).

Responsibilities

Provide Skilled PT care: in-home evaluations and individualized plans of care for geriatric patients.
Implement therapeutic interventions: functional strengthening, gait training, balance, and fall risk reduction.
Manage caseload: schedule and conduct home visits based on availability and patient needs.
Collaborate with care team to ensure coordinated patient care.
Document efficiently using EMR system to minimize documentation time.
