Job summary

Qualifications

• Current California PT license is required.
• CPR/BLS certification is required or current.
• Apartment-based or car-accessible travel to patient homes.

Responsibilities

• Provide skilled in-home PT evaluations and develop individualized geriatric plans of care.
• Implement functional strengthening, gait training, balance, and fall risk reduction.
• Schedule and conduct home visits based on availability and patient needs.
• Collaborate with the care team to ensure coordinated patient care.
