Job summary

Concierge Home Care is hiring a Home Health Physical Therapist (PT) to deliver in-home care in Marianna, FL with a flexible schedule and autonomy. You will assess patients, design personalized plans, and support recovery in a 1:1 setting.

Relocation bonus of $5,000, potential for higher earnings per visit (Medicare differential pay), and a compassionate team backing ongoing training and mentorship. In-home role with Dart-on scheduling flexibility.

Qualifications

• Active, unencumbered Florida Physical Therapist license.
• Valid driver’s license and reliable transportation.
• Flexible schedule and 1:1 patient care in home.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide therapeutic exercises and mobility training.
• Educate patients and caregivers on safety and recovery.
• Collaborate with physicians and interdisciplinary teams.
• Supervise Physical Therapist Assistants (PTAs) and complete documentation.
