Job summary

UnitedHealth Group's Geisinger Home Health is seeking a Physical Therapist to perform home assessments and develop PT plans, delivering patient-centered care in the local coverage area.

This full-time role offers flexible scheduling, a $25,000 sign-on bonus for external candidates, mileage reimbursement, and opportunities to grow within a nationwide health system.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs; assists the physician in the development of the therapy plan of care.
• Provides services within the scope of practice as defined by state laws governing physical therapy, in accordance with the plan of care, and in coordination with other health care team members.
• Observes and reports the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
• Treats patients to relieve pain, develop and restore function, and maintain maximum functioning.
• Instructs and informs health care team, patient, and family about the plan of care and progress toward goals.
• Visits patients according to Plan of Treatment and submits progress notes per policy.
• Completes all patient evaluations and develops the PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals, and anticipated projected frequency of care.
