Job summary

Concierge Home Care seeks a dedicated Occupational Therapist to deliver skilled home health services in patients' homes, helping regain independence and daily function. You will design customized plans, perform home evaluations, and work with physicians and a multidisciplinary team to enhance patient outcomes.

The role emphasizes autonomy, flexible scheduling (including PRN and contractor options), and strong use of AI-assisted documentation tools to complete visits efficiently.

Qualifications

• Active, unencumbered Florida Occupational Therapist license.
• Valid driver’s license and reliable transportation.
• CPR certification.
• Ability to supervise Certified Occupational Therapy Assistants (COTAs).
• Home health experience preferred but not required.
• Ortho experience preferred but not required.

Responsibilities

• Conduct patient evaluations and home environment assessments.
• Develop individualized treatment plans based on physician orders.
• Provide therapy focused on improving functional mobility and daily living skills.
• Educate patients and caregivers on adaptive strategies and safety techniques.
• Collaborate with physicians and interdisciplinary care teams.
• Supervise and delegate care to COTAs when appropriate.
• Document visits efficiently using AI-assisted tools.
