Responsibilities include direct care, care plan collaboration with physicians, patient education, and accurate documentation while upholding infection control and safety.

Qualifications

• Current LPN license in Michigan.
• Experience in palliative care or hospice preferred.
• Strong communication and interpersonal skills.
• Able to travel to Wayne & Oakland County.
• Work independently and as part of a multidisciplinary team.
