Background checks are required. The position involves one-on-one care in patients’ homes, coordination with a case manager, and routine documentation of care provided.

Qualifications

• Current CPR, First Aid, and 2-Step TB Test.

Responsabilités

• Executes physician prescribed plans of care and documentation.
• Administers prescribed treatments and therapies.
• Performs patient assessments and coordination of care.
• Performs activities of daily living, communications, and vitals assessment in the patient’s home.
• Communicates with family, doctors, and other caregivers about patient health.
• Performs other personal care tasks as needed.
