Job summary

Qualifications

• CPR, First Aid, and 2-step TB test required.
• Experience in clinical or home health care preferred.
• Strong interpersonal skills and physical stamina.

Responsibilities

• Administer prescribed care and document care in the system of record.
• Provide personal care and activities of daily living at the patient’s home.
• Assess patient conditions and coordinate with the case manager.
• Monitor vitals and assist with therapies as prescribed.
• Communicate with family, doctors, and care team.
