PurposeCare is a growing home health provider delivering one-on-one care to patients in their homes. You’ll help clients stay safe, comfortable, and independent while building meaningful relationships with families and teammates.

Qualifications

• Must be 18 years or older.
• At least 1 year of caregiving experience within the last 2 years under RN supervision.
• Ability to follow care plans and document accurately.
• Reliable transportation to travel to patient homes.
• Ability to lift up to 25 lbs.
• Willing to complete background check and onboarding.

Responsibilities

• Assist with personal care (bathing, grooming, dressing, toileting).
• Support mobility, transfer and ambulation.
• Provide meal prep and feeding assistance.
• Give medication reminders (no administration).
• Monitor and report changes to the office nurse.
• Maintain a clean and safe home environment.
• Document care per agency standards.
• Travel to patient homes within assigned area.
