Job summary

Coal Country Community Health Center in Beulah seeks a Float Nurse/Care Coordinator (RN or LPN) to support multiple care teams and coordinate preventive and chronic disease management services. The nurse will provide direct patient care, coordinate services, and support providers in a fast-paced environment.

Ideal candidate is flexible, patient-focused, and able to work independently while valuing teamwork to achieve quality outcomes and excellent patient experiences.

Qualifications

• Must be a graduate of an approved nursing program.
• Current, unrestricted RN or LPN license or temporary permit, required.
• Two or more years of nursing experience, required.
• Previous care coordination experience, preferred.

Responsibilities

• Coordinate patient care across multiple care teams.
• Provide high-quality, patient-centered nursing care.
• Support coverage needs and coordinate nursing team visits.
• Assist Site Coordinator and Nursing Supervisor to ensure effective care delivery.
