Responsibilities

• Provide one-on-one support to individuals with developmental disabilities in home and community
• Assist with daily living skills (cooking, cleaning, personal care, transportation)
• Encourage participation in social and community activities
• Help individuals achieve personal goals and celebrate progress
• Promote choice, independence, and self-advocacy
• Accurately document care, activities, and progress
• Communicate with team members to ensure quality support
