Job summary

Safe Life Home Health Care in Kane County, IL is seeking dedicated CNAs to join our team and deliver compassionate in-home care for our patients. You will assist with daily activities, monitor health, and support their comfort and well-being in a home setting.

The role requires an active CNA certificate, CPR, and a valid driver's license. Prior home health experience is a plus, with EMR experience in Kinnser by WellSky considered advantageous. A collaborative, empathetic approach is essential.

Qualifications

Responsibilities

• Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
• Provide mobility support for walking, transferring, and using mobility aids.
• Take and record vital signs (BP, pulse, temperature).
• Provide medication reminders and help with schedules.
• Assist with meal prep and monitoring dietary restrictions.
• Maintain patient hygiene and comfort, including linens and oral care.
• Observe and report changes in patient condition to the nursing team.
• Document care and vital signs in the EMR system (Kinnser by WellSky).
• Maintain clear communication with patient, family, and healthcare team.
• Ensure safety by following infection control protocols.
• Provide transportation to appointments as needed.
• Offer emotional support and companionship.
• Collaborate with nurses and therapists for comprehensive care.
• Educate patients and families on self-care and safety.
