Job summary

Clare Medical of New Jersey is seeking a Nurse Practitioner to perform comprehensive patient assessments, wellness and sick visits, and manage chronic diseases. You will order and interpret tests, collaborate with other providers, and prescribe therapies in a collaborative practice.

Applicants should hold an unrestricted NJ APN license with at least 1 year of clinical experience and a current DEA/CDS license. Travel within the region is required; bi-lingual skills are a plus.

Qualifications

Unrestricted APN license in NJ with at least 1 year clinical experience.
Current DEA/CDS license.
Willingness and ability to work independently.
Bi-lingual a plus.

Responsibilities

Initiate and complete detailed patient assessment & physical exams.
Wellness exams and sick visits.
Evaluate and manage common and acute chronic diseases.
Order, interpret and evaluate diagnostic and lab results.
Refer and collaborate with other providers.
Prescribe pharmacological and non-pharmacological therapies.
