Job summary

Nutrition that Works, LLC is seeking a consultant Dietitian to join our team in Lakeport, CA, serving our long-term care community for 27 hours/week. Enjoy a flexible schedule and excellent pay; drive time offered.

Responsibilities include clinical nutrition assessments, EMR documentation using the Nutrition Care Process, collaboration with the care plan team, completing the MDS - Section K, TF calculations, and wound/weight change recommendations.

Qualifications

• Credentialed by the CDR and licensed dietitian in California.
• Client-centered with strong communication skills.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Make recommendations for wounds and significant weight changes.
