Job summary

Nutrition that Works, LLC in Gridley, CA is seeking a consultant dietitian to join our team for 16 hours per week in the long-term care setting. Enjoy a flexible schedule and excellent pay.

You will complete clinical nutrition assessments, document using the Nutrition Care Process in the EMR, collaborate with the interdisciplinary care plan team, and work on MDS - Section K and wound/weight change recommendations.

Qualifications

• CDR credential required for eligibility.
• Active RD licensure required to practice.
• Strong communication and client-centered approach.
• Ability to work 16 hours/week in a long-term care setting.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Calculate TF.
• Provide recommendations for wounds and weight changes.
