Job summary

Nutrition that Works, LLC is seeking a consultant dietitian to join our team, serving the long-term care community located in Los Angeles, CA for 24 hours/week.

Enjoy a flexible schedule and excellent pay! The role includes clinical nutrition assessments, documentation via the Nutrition Care Process in the EMR, and collaboration with the interdisciplinary care plan team. Credentialed by the CDR and dietetic licensure are required.

Qualifications

• CDR credentialed and licensed as a Dietitian.
• Licensed Dietitian with CDR credential.
• Positive attitude and client-centered approach.
• Strong communication skills in interactions with patients and team.

Responsibilities

• Complete clinical nutrition assessments.
• Document using the Nutrition Care Process in the EMR.
• Collaborate with the interdisciplinary care plan team.
• Complete the MDS - Section K.
• Make recommendations for wounds and significant weight changes.
