Job summary

Concern Housing in Medford, NY is seeking a Financial Case Manager to provide financial management services to tenants in NYC housing programs, including rent collection, eligibility for benefits, and budgeting education to improve financial literacy.

You will liaise with the SSA as representative payee and coordinate with HRA, working Monday–Friday, 8am–4pm. Requires a Bachelor's in a related field and 2 years’ experience; NYS driver’s license required. Competitive benefits offered.

Qualifications

• Bachelor's Degree in Psychology, Rehabilitation, Social Work or related field plus 2 years experience in a similar setting.
• Knowledge of Social Security, Public Assistance, and Medicaid systems.
• NY State motor vehicle license required.

Responsibilities

• Provide case management services, including service planning and referrals.
• Provide individual counseling, support services, and required documentation.
• Assist clients with housing needs, landlord and lease issues, and rent related matters.
• Conduct site visits and monthly safety inspections as required.
• Assist clients with benefits, financial assistance, appeals, and recertifications.
• Coordinate Representative Payee services, including SSA documentation and benefit payments.
