Job summary

Rochester Area Neighborhood House is seeking a Case Manager to provide client-centered services for households facing financial hardship and housing instability. This full-time role focuses on helping clients move toward self-sustainability by budgeting, financial coaching, and navigating benefits and resources.

The Case Manager will assess needs, design target-based wellness plans, and collaborate with staff to support clients through outreach, documentation, and referrals.

Qualifications

• Experience in case management or related social services.
• Strong budget analysis and balancing skills.
• Excellent communication and listening abilities.
• Ability to manage multiple clients and deadlines.
• Valid driver’s license and reliable transportation if outreach required.
• Ability to work independently and as part of a team.

Responsibilities

• Conduct client assessments and develop individualized wellness plans.
• Provide short- and long-term case management based on client needs.
• Help clients address financial emergencies and improve stability.
• Assist with budgeting, prioritizing expenses, reducing financial barriers.
• Connect clients with public benefits and housing, food, employment, transportation, healthcare resources.
• Assist with benefit applications and online systems.
• Advocate for clients within complex service systems.
• Maintain confidential client records and required documentation.
• Collaborate with Neighborhood House staff and community partners.
• Participate in community outreach, resource fairs, meetings, and events.
