Job summary

Catholic Charities is seeking a Case Manager in New York to support individuals through access to services, resources, and housing stability. You will document referrals, perform initial screenings, and coordinate annual housing assessments while reporting progress and outcomes.

The role requires an Associate's Degree in Social Work (Bachelor's preferred), 1–3 years in Human Services, and a valid driver's license.

Qualifications

• Bachelor's in Social Work or related field preferred.
• 1–3 years of relevant Human Services experience.
• Valid Driver's License with insurance and access to a vehicle.

Responsibilities

• Complete documentation in the Uniform Case Record, including referrals and intake materials.
• Review intake information and perform initial screening assessments for eligibility.
• Conduct housing office intake assessments for new tenants and annual reviews.
• Develop and monitor ISP, with semi-annual progress reports and adjustments.
• Track monthly progress against housing and service milestones and targets.
