Job summary

A community support organization is looking for a Case Manager in New York to assist tenants in achieving their goals through advocacy and resource management. Qualifications

• Experience working with the Seriously and Persistently Mentally Ill (SPMI) population.
• Flexibility for occasional shift coverage until 8 pm.

Responsibilities

• Review tenant eligibility documentation.
• Conduct initial intake and assessment of tenants' needs.
• Create and maintain comprehensive tenant files.
• Prepare and adjust independent living plans with tenants.
• Assist tenants in accessing community resources.
