Job summary

AccentCare is seeking a Field Supervisor to lead Home Care Aides and oversee the delivery of personal care services across Will and Cook Counties. The role requires evaluating care plans, training staff, and ensuring compliance with client needs and agency standards.

Responsibilities include hands-on coaching, quarterly home visits, and collaboration with Care Coordinators to maintain high-quality patient care. Travel up to 80–90% is expected, with mileage reimbursement.

Qualifications

• Strong communication, organizational, and problem-solving abilities are required.
• Must have reliable transportation and valid driver's license.
• Bilingual skills are a plus but not required.

Be the Best Field Supervisor You Can Be

If you meet these qualifications, we would love to meet you:

• High School Diploma or GED required
• 2 years of Home care, caregiving or CNA Experience
• Strong communication, organizational, and problem-solving skills.
• Must have valid driver's license, reliable transportation, and auto insurance.
• Bilingual skills a plus, but not required.

Responsibilities

As a Field Supervisor, you will:

• Provide supervision and leadership to Home Care Aides
• Evaluate the quality, effectiveness and of services that are needed for each of the clients served by HCA
• Conduct unannounced quarterly home visits with clients that have Paid Family Caregivers to ensure the plan of care is being followed and HCA's are present during the hours scheduled.
• Coach, counsel, mentor and evaluate the performance of HCA's
• Conduct home visits as necessary, to provide hands on training and assistance
• Initiate and/or participate in client staff discussion with Care Coordinator
• Investigate allegations of fraud and theft reported by HCA
• Documentation and follow up on incident reports
