Position: Field Service Supervisor (FSS)

The FSS is responsible for facilitating and overseeing Direct Care Workers (DCWs) who provide direct care services to our clients. The FSS ensures that DCWs are trained on the client Plan of Care (POC) to deliver high-quality and efficient healthcare and supportive services. The FSS will also provide self-management goals and referrals to both internal and external resources. Additionally, the FSS is responsible for training, supporting, and developing all FSS staff. The role includes reviewing EVV data to ensure task completion and monitoring shift clock-ins and clock-outs.

Primary Job Responsibilities:
  1. Assist with monthly Client QA visits and complete client surveys on caseload.
  2. Update current DCW schedules with Home Care Clients.
  3. Organize and conduct Wellness Check-ups for clients, as assigned by the DFO or requested by the Director.
  4. Manage shift coverage by finding or providing coverage as needed.
  5. Track overtime usage and coordinate with the Payroll Specialist for approval.
  6. Oversee and track client referrals and onboarding of potential clients.
  7. Manage the on-call phone and assign coverage as needed.
  8. Update DCW schedules as required.
  9. Assist with conducting intakes for new clients.
  10. Identify and connect patients with community resources to facilitate referrals and address social service needs.
  11. Assist with on-call phone rotation.
Job Skills:
  • Strong written and verbal communication skills.
  • Ability to work effectively in a team.
  • Ability to follow through with delegated tasks.
  • Proven skills in data analysis, organization, and problem-solving.
  • Intermediate proficiency in Microsoft Office applications: Excel, Word, Outlook, PowerPoint.
  • Strong attention to detail and follow-up.
  • Ability to maintain client confidentiality and adhere to HIPAA laws.
