Qualifications

• Graduate of an approved school of professional nursing and current RN license in practicing state.
• One or more years of experience in clinical home health services or equivalent preferred.
• Licensed driver with transportation and insurance coverage.

Responsabilités

• Follows the Care Plan in coordination with nursing leadership and care team.
• Conducts on-site orientation, supervision, and performance of home care aides per regulations.
• Provides ongoing instructions to aides on safety, HIPAA, and client respect.
• Reviews and orients Personal Care Plans from ASAP to align with the care plan as authorized.
• Documents services rendered and changes in client status.
• Communicates concerns with ASAP Case Managers or family as needed.
• Performs additional duties as assigned.
