Job summary

Associated Home Care is seeking a Registered Nurse to administer care plans and monitor patient health in a home-health setting. You will ensure compliance with nursing standards and coordinate with the care team to support seniors in their homes.

The role requires RN licensure in the state of practice, driving with reliable transportation, and 1+ year in clinical home health. CPR certification is preferred, with full-time, part-time, or per diem options available.

Qualifications

• RN license in practicing state.
• Graduate of an approved school of professional nursing.
• Must be a licensed driver with available independent transportation and current insurance.
• CPR certification preferred.

Responsibilities

• Follows the Care Plan in collaboration with the nursing team.
• Conducts on-site orientation, quarterly supervision visits, and staff evaluations.
• Provides ongoing instructions to home health aides and reinforces safety protocols.
• Reviews care plans and orients aides per ASAP requirements.
• Documents services rendered and client condition changes.
• Communicates concerns to ASAP Case Managers or family as needed.
• Performs other duties as assigned.
