Associated Home Care in Massachusetts is seeking a Licensed Registered Nurse to administer care plans and support seniors in their homes, ensuring compliance with federal, state, and local regulations. This role focuses on delivering high-quality skilled nursing services in a home setting.

Qualifications

• Graduate of an approved school of professional nursing.
• Current licensed Registered Nurse in practicing state.
• One or more years of experience in clinical home health services or equivalent preferred.
• Must have reliable independent transportation with current insurance coverage.
• Preferred CPR certification.

Responsabilités

• Follows the Care Plan in collaboration with the Director of Nursing, patient/caregivers, physician and care team.
• Performs on-site Orientation, quarterly Supervision Visits, Skill Checks and Annual performances of home care aides in compliance with federal/state regulations and AHC policies.
• Provides ongoing instructions to home health aides during field visits including reinforcements of safety precautions and patient respect.
• Reviews Personal Care Plans from ASAP, orienting aides in following the care plan as authorized by the ASAP RN.
• Documents services rendered and changes in client conditions and/or family/home situation.
• Communicates with ASAP Case Managers or client’s legal representative/family regarding client status as necessary.
• Performs other duties as assigned.
