Job summary

Associated Home Care in Massachusetts is seeking a dedicated RN to administer the Care Plan and oversee in-home nursing services for seniors in East Longmeadow, Springfield, with Greenfield as needed. You will ensure compliance with federal/state regulations and coordinate with physicians and the care team.

The RN will follow care plans, perform orientation and supervision of home health aides, document changes, and communicate concerns to Case Managers while maintaining high-quality,

Qualifications

• Graduate of an approved school of professional nursing.
• Current licensed Registered Nurse in practicing state.
• One or more years of experience in clinical home health services, or equivalent preferred.
• Must be a licensed driver with transportation and current insurance coverage.
• Preferred CPR certification.

Responsibilities

• Follows the Care Plan in collaboration with the Director of Nursing and care team.
• Conducts on-site orientation, quarterly supervision, skill checks and annual performances of home care aides.
• Provides ongoing instruction to home health aides during visits and reinforces safety and privacy standards.
• Reviews Personal Care Plans and orients aides to follow them as authorized.
• Documents services rendered and changes in client conditions.
• Communicates concerns or issues to ASAP Case Managers or family as needed.
• Performs other duties as assigned.
