Job summary

Ohio Living in Greater Youngstown is seeking a compassionate Registered Nurse for Home Health & Hospice field staff. You will assess patients in their homes, develop and implement care plans, and coordinate with the IDG and team members to ensure quality care.

Requires Ohio RN license and at least 1 year of acute care or med-surg experience; Medicare-certified home health preferred. Proficiency with EMR and MS Office; valid driver's license; willingness to travel.

Qualifications

• Current unencumbered Ohio RN license.
• At least 1 year of acute care or med-surg nursing experience.
• Experience in a Medicare-certified home health agency preferred.
• Proficiency with Windows and MS Office; EMR experience preferred.

Responsibilities

• Care Management and Service Delivery in patients' homes or facilities; develop and monitor care plans.
• Coordinate care with IDG and health team; document and communicate changes in condition.
• Ensure compliance with federal, state, and local guidelines and agency protocols.
