We are seeking a Registered Nurse (RN) for our Georgia team to perform clinical home visits, assess needs, and develop service plans in Adult Foster Care. This field-based role requires travel to patients' homes in Douglasville, GA, with responsibilities including care planning, documentation, and coordination with

Qualifications

• Active RN license in Georgia.
• Minimum of 2 years clinical experience; 1 year with elders or adults with disabilities.
• Bilingual in Spanish is a plus.
• Experience with CRM systems (Salesforce is a plus).
• Valid driver's license with insured vehicle for travel within the state.
• Strong communication and interpersonal skills.
• Excellent written and verbal communication; problem-solving and decision-making abilities.

Responsibilities

• Provide support to the caregiver and member and review services per Adult Foster Care scope.
• Uphold client privacy and adhere to ethics and agency policies.
• Follow FreedomCare's clinical policy and procedures.
• Assess and reassess client needs and PCP forms; communicate changes.
• Develop and update the care plan with physician's plan and provide caregiver instruction.
• Complete all client documentation accurately and timely.
• Coordinate care with physicians, RNs, program director, and other resources.
• Explain agency philosophy to clients and families.
• Perform other duties as assigned.
