Job summary

FreedomCare in Georgia is seeking a Care Manager to oversee patient care in the home, perform orientations and assessments, and train caregivers across Dekalb & Rockdale, GA. The role requires fieldwork within the state and close collaboration with care teams to ensure independence and quality of care.

The ideal candidate brings 2+ years in frail/elderly care, knowledge of ADL/IADL, and a valid driver’s license for state travel.

Qualifications

• 2+ years of experience with frail or elderly adults and caregivers.
• Field experience in medical or case management preferred.
• Experience with data input and case note entry; Salesforce is a plus.
• Home care experience preferred.
• Knowledge of ADL/IADL assessments.

Responsibilities

• Conduct home orientations and sign paperwork.
• Perform initial assessments during orientations.
• Carry out periodic supervisory visits for on-care cases.
• Train caregivers and patients as required per case.
• Interact with caregivers and other stakeholders as needed.
• Participate in interdisciplinary meetings with care managers, caregivers and patients.
• Collaborate in care practice and communicate impressions to staff.
• Assess procedures and implement improvements; provide training to caregivers.
• Travel within Georgia (Atlanta Metro Area).
