Job summary

FreedomCare seeks a dedicated Care Manager to join our Georgia team, coordinating home-based care for frail or elderly patients. You will conduct orientations, assess patients, and train caregivers to ensure high-quality service for loved ones at home.

The role requires fieldwork across Georgia, strong communication, organizational, and problem-solving skills, and a valid driver’s license. Join us to support independence and compassionate care.

Qualifications

• 2+ years of experience working with frail or elderly disabled adults and caregivers.
• Field experience in a medical or case management environment is preferred.
• Experience with data input, case review/case note entry; Salesforce experience is a plus.
• Hands-on experience in home care, preferred.
• Working knowledge of ADL/IADL assessments.
• Valid driver’s license with an insured vehicle for travel within the state.
• Excellent verbal and written communication, organizational and time management skills.
• Strong critical thinking and problem-solving skills.
• Proficient in Microsoft Word, Excel, Outlook.
• Excellent training skills and a team-player mentality.

Responsibilities

• Conduct home orientations and sign paperwork.
• Perform initial assessments during orientations.
• Carry out periodic supervisory visits and case check-ins.
• Train caregivers and patients as required per case.
• Interact with caregivers and other stakeholders as needed.
• Join interdisciplinary meetings with care managers, caregivers, and patients.
• Share impressions of cases with staff and teams.
• Critically assess procedures and implement improvements for case evaluation, feedback, and caregiver training.
• Wear multiple hats as operations evolve.
• Travel within Georgia (Atlanta Metro Area).
