Qualifications

• 2+ years of experience with frail or elderly disabled adults and caregivers.
• Field experience in a medical or case management environment is preferred.
• Valid driver’s license with an insured vehicle for travel within Georgia (GA).

Responsibilities

• Conduct home orientations and sign paperwork.
• Perform initial patient assessments.
• Carry out periodic supervisory visits and check-ins for on-care cases.
• Train caregivers to ensure quality service.
• Collaborate with caregivers, patients, and staff in interdisciplinary meetings.
