Job summary

Elevance Health seeks a LTSS Service Coordinator - RN Clinician to operate primarily in the field, traveling to client sites and designated locations across New York. The role includes developing individualized care plans, coordinating with the health care team, and ensuring access to services with occasional office attendance as needed.

The candidate should have an unrestricted RN license and at least 3 years of experience with individuals with chronic conditions.

Qualifications

• Requires a HS diploma or equivalent and minimum of 3 years of experience with individuals with chronic illnesses, co-morbidities, and/or disabilities in a service coordination or similar role.
• Current, unrestricted RN license in applicable state(s) required.
• May require state-specified certification based on state law and/or contract.

Responsibilities

• Identifies members for high-risk complications and coordinates care with the health care team.
• Manages members with chronic illnesses to ensure cost-effective utilization of health benefits.
• Obtains member history and develops an individual care plan.
• Establishes goals with the member and caregivers, identifying need for alternative care or waivers.
• Develops and oversees the care plan and ensures access to services.
• Facilitates authorizations/referrals within benefits structure or via extra-contractual arrangements.
• Interfaces with Medical Directors and Inter-Disciplinary Teams on care plans.
• Assists in problem solving with providers, claims, or service issues.
• Directs and supervises non-RN clinicians in coordinating services for the member.
