Job summary

Tennessee Quality Care Hospice is seeking an RN Case Manager to join our growing Waynesboro team. This field-based role manages a patient/family caseload, coordinates care, and supports patients at home with compassion.

Responsibilities include comprehensive assessments, care planning, interdisciplinary coordination, and timely documentation to ensure high-quality, respectful end-of-life care for diverse communities in TN. On-call duties apply.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as an RN in the state of practice (TN).
• Current CPR and First Aid certifications.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance.

Responsibilities

• Assess and manage patient/family caseload through the nursing process (assessment to evaluation).
• Obtain data from patient factors and initiate intervention with the interdisciplinary team.
• Coordinate care by communicating with attending physician and other agencies.
• Provide regular visits to reassess needs and update the plan of care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records and timely care plans.
• Supervise delivery of care by Home Health Aide per guidelines.
