Saint Cyr Partners seeks an experienced Home Health RN Case Manager for a field-based role serving Fort Worth, TX and the surrounding Tarrant County area. This position requires an RN with recent home health case management experience, strong assessment skills, and the ability to manage an assigned caseload independently in the field.

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience.
• Strong understanding of Medicare home health, OASIS, and plan-of-care management.
• Ability to work independently with strong communication.
• Valid driver’s license and ability to travel locally.

Responsabilidades

• Manage assigned home health caseload in the field.
• Perform skilled visits, assessments, evaluations, recertifications, discharges.
• Develop care plans with physicians, patients, families, and team.
• Complete accurate clinical documentation including OASIS.
• Coordinate care with therapies, social work, and leadership.
• Ensure compliance with regulatory standards.
