Qualifications

• Active Texas RN license in good standing.
• Recent Home Health RN Case Management experience.
• Strong Medicare home health knowledge, OASIS, and plan-of-care management.
• Ability to work independently with strong communication.
• Valid driver’s license and ability to travel locally.

Responsibilities

• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, recertifications, and discharges.
• Develop, update, and coordinate individualized plans of care with the care team.
• Complete accurate clinical documentation, including OASIS and visit notes.
• Monitor patient progress and communicate updates to the team.
• Educate patients and caregivers on disease management and plan-of-care goals.
• Coordinate care with therapy, social work, physicians, and leadership.
• Maintain compliance with home health regulatory, documentation, and quality standards.
