• Manage an assigned home health patient caseload in the field.
• Perform skilled nursing visits, assessments, and recertifications.
• Develop and coordinate individualized plans of care with the team.
• Complete timely clinical documentation, including OASIS and visit notes.
• Monitor progress and communicate updates to the care team.
• Educate patients and caregivers on disease management and safety.
