• Experience conducting assessments and SOC - OASIS is highly desired.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Make home visits to patients in designated territory.
• Collaborate with physicians and agency staff to deliver the best care possible.
