Job summary

Alignment Health is seeking a Care Anywhere (CAW) NP/PA to perform in-home assessments and deliver comprehensive medical and social support to our patients in the Sacramento area.

As part of a multidisciplinary team, you will conduct health screenings, review medications, identify needs, and coordinate with PCPs and specialists to optimize care while reducing hospitalizations. A CA NP/PA license and experience with geriatric populations are preferred.

Qualifications

• Master’s degree from an accredited NP or PA program.
• Active CA NP or CA PA license required.
• Experience with geriatric or home-based care preferred.

Responsibilities

• Perform in-home assessments and medication reviews for Alignment members.
• Educate patients and families to support self-management at home.
• Identify diagnoses to be reviewed in care management and plan care accordingly.
• Coordinate with PCPs, specialists, and care teams to develop care plans.
• Lead clinical team discussions including nurses and social workers.
